Re-evaluations are necessary to make sure you qualify to keep receiving benefit services. When your case is coming up for re-evaluation, the Department will send you a notice with instructions for completing the re-evaluation process. Here's what you need to know to avoid any interruptions or delays in your benefit services.

Complete your re-evaluation
Follow the instructions on the re-evaluation notice we sent you, which may include completing a form, sending verifications, or both. If you lost your notice or did not receive one, call us at the number below and we'll tell you what you need to do.
If your notice instructed you to simply mail verifications to the Department, send in the requested verifications, then call the Department in a few days to complete your interview.
If you lost your re-evaluation paperwork for Food Stamps and your re-evaluation does not require an interview, you can download a blank copy of the re-evaluation paperwork.
If you lost your re-evaluation paperwork for Medicaid, or your Food Stamps re-evaluation requires an interview, you can download a blank copy of the re-evaluation paperwork.
