Provider Toolkit: Developed by the Idaho Opioid Misuse and Overdose Strategic Planning Group, the toolkit provides Idaho-specific data as well as tips and tools for the Idaho PMP, prescribing naloxone, MME calculations and more.
CDC Checklist and Oregon Pain Guidance Group Checklist: Checklists for safe and effective prescribing of opioids for chronic pain.
MATxAPP: MATx is a free mobile app that empowers health care practitioners to provide effective, evidence-based care for people with opioid use disorder. The app supports practitioners who currently provide medication-assisted treatment (MAT), as well as those who plan to do so in the future.
ASBI: Alcohol Screening and Brief Intervention for People Who Consume Alcohol and Use Opioids.
Opioid Risk Tool: The Opioid Risk Tool (ORT) is a six-question, self-administered assessment that can help predict a patients' risk level of exhibiting behaviors associated with opioid misuse or addiction.
SBIRT: Screening, Brief Intervention, and Referral to Treatment (SBIRT) is an approach to the delivery of early intervention and treatment to people with substance use disorders and those at risk of developing these disorders.
DAST and DAST-10:Drug Abuse Screening Test (DAST) is a self-report screening tool for the abuse of drugs other than alcohol. The DAST-10 is a ten item version of the screening tool.
Sample Patient Agreement Forms: The National Institute on Drug Abuse provides two patient agreement forms that can be used with patients who are beginning long-term treatment with opioids or other controlled substances.
Pocket Guide for Tapering Opioids: The CDC developed a pocket guide for providers to reference when tapering to a reduced opioid dosage or tapering and discontinuing opioid therapy.
Quality Improvement and Care Coordination Toolkit: The CDC developed a provider-focused quality improvement (QI) and care coordination toolkit for healthcare systems and practices in incorporating the CDC Guideline for Prescribing Opioids for Chronic Pain. The toolkit contains examples of existing materials, resources, and tools used by other practices.
