Immunization and Idaho Schools

The Idaho Immunization Program provides support for schools throughout Idaho by:

  • Assisting school personnel in accessing and organizing students’ immunization records
  • Saving time tracking down students’ records from healthcare providers 
  • Providing access to a statewide immunization registry housing children's immunization records 
2009 School Immunization Report

To complete your 2009 School Immunization Report, please login to IRIS at https://iris.idaho.gov. If you do not have access to the IRIS School Module please provide the IRIS School Module enrollment information below. If you need assistance completing the School Immunization Report please see below for online training opportunities and detailed instructions.

Dear School Nurses, Secretaries, and Administrators:

It is that time of year again, the 2009-2010 school year is here!  We recommend that you complete the School Immunization Report online at https://iris.idaho.gov/irisweb/main.jsp. Instructions for completing the web-based report and obtaining your IRIS username/password are enclosed. For those without internet access, please complete the enclosed paper School Immunization Report and submit by fax at (208) 334-4914. We encourage you to keep a hard copy of your final report for your records. Please complete the online or paper report no later than November 1, 2009.

It is important to review records for all students attending your school to ensure compliance with Idaho’s School Immunization Requirements. According to Idaho Code students attending a public, private or charter school in Idaho must meet minimum immunization requirements or have a valid medical, religious or personal exemption form on file at registration and before attendance to school (IDAPA 16.02.15). The minimum school immunization requirements are as follows:

Immunization Requirement  Child born after September 1, 1999  Child born on or before September 1, 1999
Measles, Mumps, and Rubella (MMR)  2 doses  1 dose
Diphtheria, Tetanus  5 doses*  4 doses
 Pertussis  5 doses*  4 doses
 Polio  3 doses  3 doses
 Hepatitis B  3 doses  3 doses

* 4 doses of DTaP meets requirements if at least 1 dose was given on or after the 4th birthday

Hepatitis B: 3 doses are required unless the child was born on or before November 22, 1991

Schools are required to report the Varicella, Hepatitis A, Td/Tdap booster, and Meningococcal vaccination status of students as detailed on the survey. Varicella, Hepatitis A, Td/Tdap booster, and Meningococcal vaccine are not required for school entry in Idaho, but are recommended by national experts for school-aged children.
