Clinical Laboratory Registration and Inspection

The Laboratory Improvement Section provides registration and inspection of clinical laboratories. The clinical laboratory inspections are performed as part of the Centers for Medicare and Medicaid Services (CMS) Clinical Laboratory Improvement Amendment (CLIA) program. The CLIA regulations can be found at the CMS website. If you have questions, please call 208-334-2235 ext. 245. Labs beginning operation must complete and submit the following forms:

X-ray Equipment Registration and Inspection


The Laboratory Improvement Section of the Bureau of Laboratories provides the inspection program for X-ray producing machines. Inspections are provided under two programs:

    • Mammography facilities are inspected under an agreement with the FDA to perform federal inspections of Idaho mammography sites.
    • All other x-ray facilities (e.g. medical, dental, veterinarian, chiropractic, academic, and industrial) are inspected based on Idaho Radiation Control Regulations.
