Talk to your doctor first, before you seek any other health care

Joining Healthy Connections?

•  If you already have a doctor, you may continue to go to that doctor. 

•  If you don’t have a doctor, you must choose a Healthy Connections doctor, or one will be assigned to you. 

•  You may choose to have a different Healthy Connections doctor for each family   member. 

•  Your Healthy Connections doctor will provide all of your primary health care needs  and refer you to a specialist when necessary. 

•  You can change your doctor by calling your local Healthy Connections contact by the twentieth of the month. The change will be effective the first of the next month.
