Healthy Connections is a mandatory Primary Care Case Mangement program for Idaho Medicaid.  Most people participating in either the Medicaid Basic or Enhanced Benefits Plan must enroll in Healthy Connections.  Healthy Connections enrollment means you choose one doctor or clinic who will manage your healthcare.
