The Idaho Division of Medicaid has a new Chronic Disease Management Pay-for-Performance Program. This program was established to develop a collaborative model between the division and the providers of Medicaid services to progressively improve the delivery of healthcare to Medicaid participants with diabetes. The program uses evidence-based quality improvement measures of provider-related activities to improve the care of participants with diabetes. Data is reported annually, and is anticipated to be a focus of provider interventions to improve care.
Overview
This program offers incentives to participating Healthy Connections providers for efforts to improve quality of care for their diabetic patients. The current program includes 10 federally qualified healthcare centers as pilot practices. Idaho Medicaid staff meets regularly with representatives to discuss efforts to improve diabetes care and feedback on how the program is working. The program will eventually be opened to all Healthy Connections providers who wish to participate.
