  • Ambulatory Surgery Centers
  • Hospitals
  • Nursing Facilities
  • Intermediate Care Facilities for Individuals with Intellectual Disabilities
  • End Stage Renal Disease Facilities

Electronic Submission of Reportable Events - Now Active - Thursday, May 26, 2016

We are actively working on the establishment of a portal that will allow the Long Term Care provider community to electronically report events, as outlined in BFS Informational Letter 2014-04, dated May 23, 2014.  Once implemented, the replacement system should greatly reduce the paper-burden associated with the reporting and enable the facilities to better track submitted items.  Our goal is to implement the system in June or July 2016.

The Portal is now active. To learn more and to use the portal see the Reporting Events module on right side of this page.

Please Note: this portal is to be used to report incidents as well as the results of the facility's investigation of those events.

All facilities should be accessing this system now.  As of November 1, 2016, all facilities need to use the portal to report events and to upload investigation results.  After the deadline, the call-in line will be removed and the facility will no longer fax in results of investigations.
