The Family Planning, STD and HIV Programs contract with agencies such as District Health Departments and community-based organizations to provide HIV prevention activities.

State of Idaho contractors receiving HIV prevention funds are required to collect data for contract monitoring and evaluation purposes. The provider toolbox below offers links to required monitoring forms for all HIV prevention intervention categories including Health Education and Risk Reduction group- and individual-level interventions, PCM, and Outreach.
