The Medicaid Provider Handbook is a resource tool for billing guidelines.  It is currently available on the PES software CD provided to providers by the Division of Medicaid and in hardcopy.  You may obtain a copy by contacting EDS Provider Services at: (800) 685-3757 or in the Boise area at: (208) 383-4310.
