Applying for Developmental Disability Services

Step 1: Fill out the Application for Assistance

The first step in applying for DD services is to fill out the Application for Assistance. Through this application you can apply for the following services for children and adults with developmental disabilities:

  • Health Coverage (Medicaid) - The Medicaid Enhanced Plan is the plan for children and adults with disabilities or special health needs. You can learn more about Medicaid by reading the Idaho Health Plan Coverage booklet.
  • Cash Assistance - for emergency situations, families with children, and the elderly, blind, or disabled. 

Step 2: Fill out the Eligibility Application for Adults with Developmental Disabilities

Once you have been verified to be eligible for Medicaid you can then fill out the Eligibility Application for Adults with Developmental Disabilities to apply for specific services including DD/Waiver services (traditional or self-directed community supports), Developmental Disabilities Agencies (DDA) services, service coordination, and/or Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/ID). Step 1: Fill out the Application for Assistance

The first step in applying for DD services is to fill out the Application for Assistance. Through this application you can apply for the following services for children and adults with developmental disabilities:

Step 2: Fill out the Children's Developmental Disabilities Application

Once your child has been verified to be eligible for Medicaid, you will need to fill out the Children's Developmental Disabilities Application and send it to your regional Family and Community Services (FACS) Developmental Disability Program. If you have any questions or need more information, please contact your regional Family and Community Services Developmental Disability Program or call 208-334-5512. Once you have completed and returned the application, the regional FACS staff will forward it to your Independent Assessment Provider (IAP) who will review it and contact you for more information or to schedule an appointment.

 

In order to apply for adult developmental disability (DD) services, you must first be determined to be financially eligible for Medicaid by filling out the Application for Assistance. See the chart below:
