Healthcare Directive Registry Form

This form is for individuals that previously registered their Advance Directive and/or POST with the Secretary of State.
Advance Directive documents registered at the Secretary of State were transferred to the new Idaho Healthcare Directive Registry at the Department of Health and Welfare. Please fill out this form to create your account in the new registry. This will allow you to access your existing Advance Directive and use new registry features, such as sharing your directive with individuals you choose.
