Idahoans can view, download, and share their immunization records from Idaho's Immunization Reminder Information System (IRIS) using a health record mobile app called Docket®.
Docket® is an application that searches Idaho’s Immunization Reminder Information System (IRIS) using basic demographic information (name, date of birth, and sex) to display immunization record information on a mobile device. Phone number verification is required. The app displays vaccination history to anyone with a record in IRIS whose identity has been verified through the Docket® app. This application is not a vaccine passport. The Docket® app currently supports a QR code for COVID-19 vaccinations.
Anyone at least 18 years of age with a vaccine record in IRIS can use the Docket® app to review their own immunization record or to review the immunization record of someone for whom they are the parent/legal guardian or authorized personal representative, usually for a minor child or vulnerable adult.
Use Docket® whenever you need to share immunization records, such as during school registration or child care registration.
Using Docket® to view your immunization record is easy. To get started, download the free app by searching "Docket®" on the Apple App Store or Google Play. Or go to Docket® to download the app.
1. Set up your account
Download Docket® to your mobile device for free from the Apple App store or Google Play store. Log in with an email, Apple, or Google ID.
2. Search your immunization records
Select 'Immunizations' from the main menu. Anyone 18 years old or older with a current phone number in IRIS can access their records.
3. Use Docket® when you need to access your records
You can access your and your minor child’s official immunization records directly with Docket®. You can export PDFs of your records by clicking the “share” button and clicking “Download PDF” within the Docket® app. You can then show or send the PDF as you choose.

Using the Docket® app
There are different reasons that could prevent you seeing your records through Docket® such as having multiple possible matches in IRIS, misspelled names, or a previous phone number in your IRIS record. Visit the Idaho Immunization Program (IIP) No Match Found Form to notify the support team. You’ll receive a response within five business days.
Try searching using multiple combinations of your name, with and without the punctuation. If you are unable to access your record via Docket®, submit a request through the No Match Found Form to notify the support team. You’ll receive a response within five business days.
Contact your healthcare provider to have them correct the information in IRIS, which will be reflected in Docket® as soon as changes are made.
Although your phone is prompting your 11-digit number for verification, Docket® will only accept a 10-digit phone number.
Go to your device’s settings app and reduce font/ text size. The most likely cause is that the screen is not allowing for all 8 spaces to be displayed.

Once your child is a legal adult (on their 18th birthday), you will no longer be able to access their record. However, Docket® makes it easy for your child to access their own record. Other ways for your adult child to obtain this information are to request them from their physician or by submitting a Record Request to the Idaho Department of Health and Welfare.
Data and privacy
The data are what is contained in Idaho’s Immunization Reminder Information System (IRIS), a secure statewide immunization information system and registry. Data include limited demographic information (e.g., name and phone number), details about what vaccines have been received and when, and whether any vaccines are currently due. Users can delete their search history, which deletes the corresponding records from Docket®. Users can also delete their accounts, which deletes everything associated with the account.
Your personal and health data are securely stored and securely transmitted. Access is only allowed to someone that has verified they have the legal authority to access a specific immunization record.
