M-CHAT-R Questionnaire (English)

If your child is between 16 and 30 months of age and you have concerns with your child’s behaviors or their ability to interact with you or others, we recommend completing the M-CHAT. Once submitted, this questionnaire will be sent to trained staff who will contact you to follow-up with results.
