To enter the My Family. My Story. Art and Expression Series, you can submit your artwork or expression and consent forms online or by mail. For online submission, fill out the form below and upload your artwork and consent form available in "Get Entry & Consent Forms." For a mail-in submission go to the "Get Entry & Consent Forms" to download the form, fill it out and mail the form to the address listed.
Entry forms will be accepted year-round, but to be entered in the current year's series, they must be postmarked by August 18, 2023.

Series Information
Entry forms must be postmarked by August 18, 2023. All entries will be entered into a raffle for cash and prizes. The Department would like to acknowledge and thank our partners at Casey Family Programs. Through our partnership and their generosity, we are able to provide prizes for the contest.
