ADRD Hospital Form

We have just completed our first “official” data collection quarter for the new Crisis Response for Persons with A Neurocognitive Disorder law, 56-2101. Per the Idaho Code, the reports are due to the Department by April 30, 2025

We changed the reporting form to better serve hospitals across the state. We are now using the Idaho Resource Tracking System (IRTS). If you or your team still do not have access to the IRTS platform to access the report and submit your data, please email (name, email address, and phone number) at adrd@dhw.idaho.gov so that we can add you.

When logging into the IRTS, you will need to look for the ADRD report. ADRD stands for Alzheimer’s Disease and Related Dementias. Select the Menu, then View, then ADRD.
