Families visiting this page can apply for the Developmental Milestones Program.
To sign up for the Developmental Milestones program, complete a questionnaire either online or by mail to tell us what your child is doing and learning. You'll then be offered questionnaires every few months as your child reaches new milestones. The questions cover your child's communication, gross motor, fine motor, problem solving, and social-emotional skills.

We will share the results of your questionnaire with you by email, phone, or mail. If your child's scores show that he or she could benefit from a free developmental evaluation, we may offer one through the Infant Toddler Program. The program is voluntary. You choose what you think will work best for you and your child.
