Developmental Milestones Questionnaire
The Developmental Milestones program invites you to complete an Ages & Stages Questionnaire (ASQ) to tell us what your child is doing and learning in the areas of communication, gross motor, fine motor, problem solving, and social-emotional skills. We recommend completing a new questionnaire for your child every 4-6 months or any time you have concerns about their development.

The Modified Checklist for Autism in Toddlers (M-CHAT) is a parent-report screening tool to assess the potential likelihood for Autism in children between 16 and 30 months of age. If you have concerns with your child’s behaviors or how they interact with you or others, we recommend completing the M-CHAT.

What happens after I submit a questionnaire?
After the questionnaire has been submitted, it will be reviewed by trained staff. We will share the results of your questionnaire with you by email or phone. If your child's scores show that he or she could benefit from a developmental evaluation, we may offer one through the Infant Toddler Program. This developmental evaluation would be provided at no cost to you, and the program is voluntary. You choose what you think will work best for you and your child.
