DUI evaluations must be completed by a substance use disorder service provider approved by the Department of Health and Welfare.
To find a provider who offers DUI evaluations, please contact your district court by county using the list below.
Approved substance use disorder providers can perform alcohol evaluations if they meet the following criteria:
- They are working within the scope of their license/credentials.
- They hold a license/credential listed under qualified Substance Use Disorders professional in IDAPA 16.07.17.200.
DUI evaluation requires the use of at least three screening instruments approved by the Department of Health and Welfare.

Specific requirements related to DUI evaluations are managed by the district courts. Local courts may have agencies they prefer to work with; it is the responsibility of the person seeking an evaluation to communicate with the district court to select a DUI evaluator approved by the court. District courts will also be the contact for any DUI or DUI evaluation questions.
