School and Daycare Information

The Idaho Immunization Program provides support to school and daycare facilities that have questions about immunizations.

Immunization requirements

For Schools

To enter or transfer into public, private, or parochial schools, all children in preschool and grades K-12 must provide immunization documentation or have an exemption on file (Idaho Statute 39-4801). Children who do not have all required immunizations but plan to get caught up may be conditionally enrolled.

For Daycares

In order for children to attend a licensed daycare facility, parents must provide proof of immunization status or have an exemption on file (Idaho Statute 39-1118). Children who do not have all required immunizations but plan to get caught up may conditionally attend.

Conditional Admission/Enrollment

Children who are in the process of receiving required immunizations are permitted to attend preschool, grades K-12, or daycare. Parents should sign a Conditional Admission form (for preschool and K-12), or one similar, to indicate they are in the process of having their child receive the required immunizations.

Exemptions

Idaho law allows a parent or guardian to claim an immunization exemption for their child for medical, religious, or other reasons. A medical exemption must be completed by a licensed physician. physician’s representative, or another licensed healthcare professional. Exemptions for religious or other reasons may be documented on the exemption form provided by Idaho Immunization Program. Parents or guardians may also claim an immunization exemption by providing a signed written statement.

Idaho law also requires school officials and daycare facilities to describe the exemptions provided in section 39-4801 and 39-1118 and provide a citation to the section in any communication to parents and guardians regarding immunization.

School Immunization Report

Idaho schools are required to submit kindergarten, first grade, seventh grade, and twelfth grade immunization status data to the Idaho Immunization Program by November 1 of each school year. Data must include the number of students with an up-to-date immunization record, the number of students conditionally admitted, the number of students with exemptions, the number of students that do not have all required immunizations, and the number of students with no immunization record on file. The Idaho Immunization Program compiles these data and makes them available each year.

Patient Access to Immunization Records

Docket® is a free application that searches Idaho’s Immunization Reminder Information System (IRIS) using basic demographic information (name, date of birth, and sex) to display immunization record information on a mobile device or online. Phone number verification is required. The app displays personal vaccination history data to anyone with a record in IRIS whose identity has been verified through the Docket® app.
