Developmental disability (DD) services are designed to provide individualized care and support to members who choose to receive services. Supports are provided in a location that is convenient to the member and make sense to the service being provided, whether that be in the member's own home or in the community. The type of service a member qualifies for is based on a functional assessment, maladaptive behavior, a combination of both, or a medical condition.
Members who qualify for traditional DD services will also qualify for services from a plan developer/targeted service coordinator. After the assessment is completed and an individualized budget is generated, the plan developer/targeted service coordinator will meet with the member and their support team and assist the member to lead a person-centered planning meeting. The plan developer/targeted service coordinator will write a plan for specific services chosen by the member that includes member-specific goals and supports.

When you choose to access traditional developmental disability services, you will select a provider that is able to administer the services that you need. Your plan developer/targeted service coordinator can help you choose the provider types and specific providers that will be able to meet your individualized needs.

Residential Habilitation Agencies (ResHab) provide supported living services, which are an integrated array of individually-tailored services and supports which are designed to assist you to reside successfully in your own homes, with your family, or in an alternate family home, if eligible.
Certified Family Homes (CFH) provide a safe, family-style living environment for you if you are an adult who needs some assistance with the activities of daily living, but doesn't require a more restrictive institutional setting
Developmental Disabilities Agencies (DDA) are certified entities open to the general public that are non-residential in nature and provide developmental therapy on an outpatient basis to qualified members with developmental disabilities. DDAs often provide adult day health within the facility as well if you are in need of that and you qualify.
An alternative to traditional developmental disability services is self-direction. If you are on the DD waiver, this option is available to you. This option gives you choices and the freedom to manage your own life. You don't have to choose supports alone. You will have as much or as little help as you need from a support broker, a circle of support, and a fiscal employer agent.
