Eligibility
To access our services, your child must have Idaho Medicaid.
To apply for DD support services, please complete the application packet. When your completed application is received, an assessor will contact you to schedule an eligibility evaluation. When your child is found eligible for a support budget, a case manager will be assigned to help you navigate options and link you to services.
